CONNECTION BETWEEN EBP AND QUADRUPLE AIM
Evidence based practice is the use of best scientific evidence, integrated with clinical EXPERIENCE and incorporating patient values and preference in the practice of professional nursing care.
Evidence based practice have continued to have ground in management, the formation of public policies, education and nursing practice since its formal introduction in 1990s. As one of the most problem solving in EBP, it is often described as an approach to patient care that involves considering the best available research and guidelines associated with specific clinical situation.
The main aim of EBP is to ensure that patient are provided with high quality and safe health care services for high chances of success. EBP embraces an interproffesional model where shared decision making is greatly emphasized. According to Crabtree, (2016), nurses have widely embraced a positive attitude towards EBP.
The quadruple aim has four major goals mainly: improving patient experience, population health, the work life of health care providers and reducing health care associated costs (Kim et al) (2016).
Quadruple aim purpose to achieve the aforementioned goals through the resigning of healthcare facilities. However a major setback is achieving these goals in the wider health sector, improving population heath while at the same time containing or reducing healthcare related costs, which require high level of efficiency and effectiveness.

IMPACT OF EBP ON QUADRUPLE AIM.
EBP has direct impact on the work life of health care providers since, to receive high quality and safe healthcare services, staff need to be very productive. Another impact is that errors in decision making become less frequent. Also nursing care is more efficient when ineffective practices are replaced.
EBP has helped reach the quadruple aim in the four measures as described below:
1. Patient experience.
All strategies used in EBP permit health care providers to translate the finding of research into clinical practice. By utilizing efficient searching skills of literature and formally applying the rules that guide the evidence to evaluate the findings of research, nurses and other healthcare providers can efficiently apply existent scientific knowledge to clinical practices based on individual patient needs. As a result this improves the quality of healthcare services that patient receive.in recent times, most health care institutions are embracing EBP to solve clinical issues that directly impact patient care and this is what has led to improved patient experience.

2. Population health.
EBP purpose to educate population on individual health determinants, the benefit and limitation of specific treatment, cultural and healthcare practices.
Those integrating EBP in various population setting also purpose to ensure that equitable distribution of resources to achieve specific aims (Sakallaris et al) (2016). Population can be classified based on its needs as identified by social economic status, the support received from the families and individual health status. This promote equity and efficient in allocation and distribution of resources for health promotions. All EBP involved in care have been specifically designed to address individual patient needs as opposed to shaping healthcare patterns based on subgroups.
3. Cost.
A major challenge for most healthcare facilities is directly related to the reduction of per capital cost of health as it needs all health care expenditures to be captured, measures of actual cost and indexing the cost to the health care market. According to (Sakallasis et al 2016), the most common method used to measure the actual cost is discounts and pricing. Therefore, for most healthcare institutions it is a huge challenge to provide safe and quality healthcare services at lower health care cost. When an institution embraces the use of latest technology to improve efficient and most recent treatment methods as revealed through EBP, the quality of care as well as the cost automatically increases.
4. The work life of healthcare providers.
The key component of conductive healthcare environment should support inter professional collaboration. Inter professional collaboration has been evidenced to improve patient halt outcomes and the satisfaction of healthcare providers. It also reduces provider’s burnout and turnover rates, which promote staff productivity. The 3 major components of such an environment include the structural and policy element and actively engaging the workforce in shared decision making.

Evidence-based practice is an approach that enables nurses to provide a solution to daily problems encountered in clinical practice. It results in improved outcomes of care hence facilitates healing. It involves accessing, summarizing, and application of information from the literature to solve daily clinical problems (Straus et al., 2018). It requires systemic observation and experience for nurses to make informed choices. Evidence-based practice enables nurses to enrich and enhance their clinical practice, experience, and training with the latest update of new research.
According to Berwick, Nolan and Whittington’s evidence-based practice had a purpose of meeting three health care goals (triple aim); to improve the patients’ experience during care that involves quality of care provided and their satisfaction. Secondly, improve the population health and lastly to reduce the health care costs to improve the accessibility to health. Recently the triple aim expanded its importance by adding one more goal being improving the work life of health providers and protecting their security (Cameron, 2017). Evidence-based practice does not only empower clinical workers resulting in high levels of job satisfaction but also help health care systems to achieve the Quadruple Aim (Ryan, 2016). Apart from its many positive effects on health care, evidence-based practice has also failed to provide standard quality of care throughout the world in the past years due to some barriers.
These barriers to the standard of care are; to begin with lack of enough numbers of evidence-based tutors who help to mentor students doing their degrees and masters in nursing. It will lead to a lack of competent health care providers since in cooperation of evidence-based practice will not be available during training and practice (Straus et al., 2018). Availability of enough mentors will enable incorporation of evidence-based practice throughout the nurses training experiences and education thus improving the work life of health providers. Evidence-based practice will allow the health care providers to adapt to situations, work collaboratively with each other and is also of importance as in the utilization of informatics. Secondly, Inadequate knowledge and skills by nurses and other clinicians about the evidence-based practice will affect the patients’ outcome negatively, unlike when nurses possess up to date skills and knowledge (O’Hare, 2015). When current health care research is used in the management of the patients, health complications that are associated with chronic illness are minimized hence improved patients outcome.
Besides, availability of tools and resources for care provision, including investment of evidenced-based practice budget by chief executive nurses will affect the costs and quality of care provided and the outcomes of the responsibility (Ryan, 2016). Involving the evidenced-based practice patient-centered approach in the management of chronic illnesses will reduce unnecessary costs and also reduce bills for healthier patients. Developed countries have enough tools and resources hence, and they have terminated most of the chronic illnesses than the developing countries that have limited resources failing to budget for evidence-based practice thus their costs of care are high. Availability of enough resources will also result in improved care outcomes, unlike when there are unlimited resources that hinder care provision.
Furthermore, continuous academic programs that offer education to nursing and medicine students of baccalaureate and masters on how to carry out research instead of teaching them on evidenced-based practice affects the population health and even the work life of health providers. Without evidenced-based practice, most of the health providers will not be confident during care provision, and this may result in poor health outcome population. Evidence-based practice will guide the health care clinicians since, with it, they will provide quality of care hence improved care outcomes (Straus et al., 2018). When nursing students learn evidence-based practice theories, they not only apply the knowledge gained in their carrier but also make use of the experience in advancing patient care.
Additionally, resistance from colleagues, nurse managers, and leaders affects the quadruple aim of evidence-based practice. Some leaders and nursing managers are always against the up to date research by denying nurses a chance to go back to schools and upgrade their education. They lack knowledge that current research will enrich the nurses hence improving their confidence during care provision, and also they will be able to provide proper care to the patients (Ryan, 2016). Lack of collaboration among health care also hinders in cooperation of up to date research in the treatment of patients since some of the nurses are old in the profession and they lack current skills in the provision of care. Some health care providers with update knowledge may also fear to practice for they fear being envied by their colleagues.
Lastly, misperceptions that evidence-based practice takes too much time profoundly affects the quadruple aim. Many health care providers a\re reluctant to provide up to date research since they view it as time wastage (Rousseau & Gunia, 2016). Most of them practice the ancient techniques since they consider updating their skills as wastage of time to earn. It affects the patients’ experiences negatively because they may fail to seek for the care in the same health facilities claiming that there is a lack of evidence-based practice.
