Nurse practitioners (NPs) are advanced practice registered nurses who provide primary, acute, and specialty healthcare to patients across settings. The role began developing in the 1960s in response to a shortage of primary care physicians (AANP, 2022). NPs work autonomously and collaboratively with other providers to examine, diagnose, and treat patients. Common responsibilities include taking medical histories, performing physical exams, ordering and interpreting diagnostic tests, diagnosing and treating acute and chronic conditions, prescribing medications, and providing health education and counseling (NCSBN, 2022).
Clinical nurse specialists (CNSs) are advanced practice nurses who provide direct patient care as well as broader roles in research, administration, and education. The CNS role emerged in the 1960s to bridge nursing practice and research (AANP, 2022). CNSs work in hospitals, community settings, long-term care facilities, and other specialized areas. Their responsibilities include direct patient care, consulting, research, and educating patients, families, nurses, and other healthcare professionals (NCSBN, 2022).
Certified registered nurse anesthetists (CRNAs) are advanced practice nurses who administer anesthesia, perform pain management, and provide pre- and post-anesthesia care. The role began developing in the late 1800s and gained momentum in the 1920s (AANP, 2022). CRNAs work autonomously or under physician supervision to plan and provide anesthesia care and pain management before, during, and after surgical, therapeutic, diagnostic, and obstetrical procedures (NCSBN, 2022).
Certified nurse-midwives (CNMs) are advanced practice nurses educated in both nursing and midwifery. The role emerged in the 1920s to address high maternal and infant mortality rates, especially in rural areas (AANP, 2022). CNMs provide primary care related to women’s reproductive health, pregnancy, childbirth, the postpartum period, care of the newborn, and the family planning and gynecological needs of women (NCSBN, 2022). CNMs work in various settings such as private offices, clinics, homes, and hospitals.
To summarize the historical development of one APRN role, nurse practitioners emerged in the 1960s in response to physician shortages, especially in primary care (AANP, 2022). NPs were educated to provide services traditionally delivered by physicians to expand access to care. Over time, the NP role has continued to evolve and take on more responsibilities to help meet the growing demand for healthcare services. NPs now practice autonomously and play a critical role in healthcare delivery.
