You will begin to take the lead in advocating for and collaborating with others to improve the health care outcomes for populations at risk.

Identification of a Population in Your Community
I live and work in a small rural community in Defiance County, Ohio. The community has the privilege of access to a healthcare facility that serves the community efficiently. I work as a Registered Nurse in the labor and delivery department. The community is majorly White and Hispanic. There is an increase in the number of Whites and Hispanics visiting the healthcare facility.
The determinants of health in the community comprise behavioral and social factors. Social factors affect the quality of life of the population. For instance, limited access to clean and fresh groceries is a major concern in the area (Kettunen et al., 2017). According to the World Population Review, the social factors include poverty since the community is living below the federal poverty level at 10.4 percent compared to 12.2 percent (US. Census Bureau, 2017). The poverty level has led to a stall in the number of children with a complete vaccine schedule. According to the Ohio Health Department, the percentage of vaccine schedule remains at 68 percent since 2009.
The population at risk include children between 0-2 years. The children are at risk due to vaccine-preventable diseases. According to Healthy People 2020, there is a need to increase the number of children receiving a vaccination (Determinants of Health, 2019). While working in the labor and delivery department, vaccination is a sensitive issue in care delivery. According to the Ohio Department of Health, only 68 percent of the population between 19 and 36 are immunized (Kettunen et al., 2017). The poverty levels, high cost of care, behavioral, and social factors may undermine access to immunization services. In the future, the late compliance with the vaccination schedules may decline or stagnate if the state health department does not address the factors affecting vaccination.
