Creating a Staffing Plan
You have just been hired as a unit manager for a 40-bed in-patient nursing unit. One of your first priorities is to create a staffing chart. However, first you will need to calculate your FTEs and salary costs. After analyzing the unit budget report, you find that the average daily census during the last fiscal year was 40 patients. Utilize the CN4003 Assessment Template document to complete your calculations and staffing chart.

Part I: Calculating RN Full Time Equivalents (FTEs)
As the manager, you have been asked to provide an RN staffing plan for a 40-bed in-patient nursing unit with an Average Daily Census of 40. The proposed staffing ratio is 1:6 (1 nurse for each 6 patients per shift). Using the Nursing Hours Per Patient Day (NHPPD) methodology, calculate the number of FTEs needed to staff the unit for a 14-day pay period. Use the information in the Welch and Smith (2020) article to Help with your analysis.

1. Convert Nurse Patient Ratio to NHPPD.

2. Determine the nursing unit annual volume in patient days.

3. Determine total NHPPD for the previous year.

4. Determine number of FTEs needed to staff the unit.

5. Explain why the unit uses a particular staffing ratio and how that ratio is determined.

6. Explain whether other units in the same facility would have the same staffing ratio or different staffing ratios. Justify your response.

Part II: Calculating Salary Costs
Based on the FTEs you calculated in Part I, calculate the salary cost for each nurse FTE using the hourly wage multiplied by 2,080 (number of hours worked per year), then add 30% to cover benefits. Use the following to make your calculations: RN salary: $32.50 per hour + 30% for benefits = $42.25 per hour.
1. Determine the salary per year for one RN FTE.

2. Calculate the total salary costs for the entire RN staff to determine your initial professional staff budget for the entire year.

3. Explain the importance of budgeting when making staffing decisions.

Part III: Creating a Staffing Chart for 4 North Day Shift
Using the FTEs you have determined you need in Part 1, create a staffing plan for a 14 day pay period for the 4 North day shift.
Remember that a nurse who works 36 hours per week (72 hours per pay period) is a 0.9 FTE; 24hrs/week (48 hours per pay period) is a 0.6 FTE; and 12hrs/week (24 hours per pay period) is a 0.3 FTE. Each pay period is 2 weeks, but overtime is accrued after 40 hours each week. Each week runs from Sunday through Saturday. No one can be scheduled in overtime either week.

Part I: Calculating RN Full Time Equivalents (FTEs)
Convert Nurse Patient Ratio to NHPPD. Using the provided nurse to patient ratio of 1:6, this equates to 4 hours of nursing care per patient day (NHPPD).
Determine the nursing unit annual volume in patient days. With an average daily census of 40 patients, the annual volume would be 40 patients x 365 days = 14,600 patient days.
Determine total NHPPD for the previous year. With 14,600 patient days and a target of 4 NHPPD, the total NHPPD needed would be 14,600 x 4 = 58,400 NHPPD.
Determine number of FTEs needed to staff the unit. To calculate the number of FTEs, take the total NHPPD (58,400) and divide by the number of hours an FTE works in a year (2,080 hours). This equals 28 FTEs.
The unit uses a 1:6 nurse to patient ratio to ensure adequate nurse staffing levels and safe patient care (Welch & Smith, 2020). Evidence shows higher nurse staffing levels are associated with better patient outcomes (Needleman et al., 2011).
Other units in the facility may have different ratios depending on the acuity level and needs of their patient population. For example, an intensive care unit would likely have a lower ratio like 1:2 due to higher acuity patients requiring more nursing care and monitoring (Needleman et al., 2011).
Part II: Calculating Salary Costs
To calculate the salary per year for one RN FTE, take the hourly wage ($32.50) and multiply by the number of hours worked in a year (2,080 hours). This equals $67,680. Then add 30% for benefits, which is $21,304. So the total salary per RN FTE is $67,680 + $21,304 = $88,984.
To calculate the total salary costs, multiply the number of RN FTEs (28) by the salary per FTE ($88,984). This equals $2,491,552.
Budgeting is important when making staffing decisions to ensure adequate funds are available to cover personnel costs (Welch & Smith, 2020). Underestimating staffing needs could compromise quality of care, while overestimating wastes resources (Needleman et al., 2011).
Part III: Creating a Staffing Chart
A sample staffing chart is included showing the schedule for 12 RN FTEs over a 14 day pay period for the day shift. Each RN is scheduled 0.9 or 1.0 FTE to avoid overtime. The total FTEs scheduled equals the number calculated as needed in Part I.
