Part 2: Disease Prevention Teaching Project

Step 1: Choose a community health agency (day care, church, school, clinic, homeless shelter).

Step 2: Review Table 1-1 (p. 49) of Community/Public Health Nursing and the results of your windshield survey completed in Week 2.

From the results of your windshield survey, you will identify a health issue facing the community. You will prepare an outline for a teaching project surrounding the issue, which you would present to your selected agency about this issue.

Enrollment in the Supplemental Nutrition Helpance Program (SNAP)
Enrollment in the Commodity Supplemental Food Program (CSFP), which provides monthly food packages for low-income seniors
Comparison of JK’s Community to County Census Data

Based on the county census data, JK’s community has a higher proportion of Black residents and residents living below the poverty line compared to the county as a whole. The community also has a higher prevalence of chronic diseases such as hypertension and asthma.

PART 2: DISEASE PREVENTION TEACHING PROJECT

Step 1: Choose a Community Health Agency

For this project, we have chosen a local church as the community health agency.

Step 2: Assess the Community

Based on our windshield survey, the following public health issue was identified as a priority for the community:

The selected agency, a local church, could address this issue by offering a disease prevention teaching project focused on promoting healthy lifestyle behaviors and connecting community members with available healthcare resources.
