Background
Costa Concordia disaster: On 13 January 2012, the Italian cruise ship Costa Concordia ran aground and overturned after striking an underwater rock off Isola del Giglio, Tuscany, resulting in 32 deaths. The eight-year-old Costa Cruises vessel was on the first leg of a cruise around the Mediterranean Sea when she deviated from her planned route at the Isola del Giglio, sailed closer to the island, and struck a rock formation on the sea floor. A six-hour rescue effort brought most of the passengers ashore.
An investigation focused on shortcomings in the procedures followed by Costa Concordia’s crew and the actions of her captain, Francesco Schettino, who left the ship prematurely. About 300 passengers were left on board, most of whom were rescued by helicopter or motorboats in the area.
Assignment brief
By using available information for the specific incident and the general literature on the safety and reliability of maritime operations and equipment, carry out a study of approximately 1200 words (+/- 10%) on the M/V Costa Concordia incident, addressing the following points:
1. Events leading up to the incident (20%)
Analysis of the sequence of events leading to the incident and its consequences; identifying design and operational failures and gaps in the existing maritime regulatory framework.
2. The actions of the Captain and the bridge team that may have reduced the likelihood of the incident occurring. (25%)
3. Discuss the findings of the incident, do you agree?
Discuss your judgment on ‘The captain is responsible for the significantly high number of deaths and injuries after the accident. He exercised poor leadership by failing to give orders as soon as he was made aware of the damages caused by the collision. (20%)
4. Discuss the industry reaction and which new polices were implemented for Safety at Sea following the incident. (20%) Format and Referencing (15%)
