Family Assessment Tools
Identify and describe a full or partial family assessment tool that may be used with your client/family.

Population:
The population chosen for the home visiting client is older adults with functional limitations. This population includes individuals who have difficulty performing daily activities and tasks due to physical, cognitive, or emotional impairments.

Priority Public Health Concerns:

Fall prevention: Falls are a leading cause of injury among older adults, and can lead to long-term disability and reduced quality of life. Public health-based strategies that can help prevent falls include home safety assessments, exercise programs, and medication management.
Chronic disease management: Many older adults have multiple chronic conditions, such as diabetes, hypertension, and heart disease, that require ongoing management. Public health-based strategies that can help manage these conditions include disease self-management programs, regular screenings, and medication management.
Social isolation: Older adults who live alone or have limited social support are at increased risk of depression, anxiety, and other mental health issues. Public health-based strategies that can help reduce social isolation include community-based programs, volunteer programs, and support groups.

Resources and data that can impact the home visiting client include information on quality of life, incomes, spending, and health. These resources and data can be found on Policy Map by searching the community and looking at data on poverty rates, education levels, and access to healthcare.

Focused Assessment Tools:

The Timed Up and Go Test (TUG) can be used to assess an individual’s risk of falls. The TUG measures the time it takes for an individual to rise from a chair, walk 3 meters, turn around, and return to the chair.
The Morisky Medication Adherence Scale (MMAS-8) can be used to assess an individual’s risk of medication non-adherence. The MMAS-8 is a self-administered questionnaire that assesses an individual’s knowledge, attitudes, and behaviors related to medication taking.
The Geriatric Depression Scale (GDS) can be used to assess an individual’s risk of depression. The GDS is a self-administered questionnaire that assesses an individual’s mood, interest in activities, and overall well-being.
Family Assessment Tools:
One example of a family assessment tool that may be used is the Family Assessment Device (FAD). The FAD is a standardized questionnaire that assesses various aspects of family functioning, including communication, problem-solving, roles and rules, affective responsiveness, and behavioral control.
