• The trajectory model is a nursing model developed by anselm l. Straus, a medical sociologist, and Juliet Corbin, a nurse theorist. • It is also known as the “corbin-strauss-model” and is recognized as a middlerange explanatory nursing theory (corbin & straus, 1991)
• This model was created after more than 30 years of interdisciplinary research on a wide range of chronic illnesses.

– • The term “trajectory” refers to the progression of a chronic disease through its various stages and phases.
Chronic disease
• Chronic illnesses are serious diseases that can affect a person’s mental, emotional, and social well-being for the rest of their lives.
The disease progresses through eight stages.
Mccorkle and Pasacreta (2001) explain the eight stages of the chronic illness trajectory.
1. The initial or pretrajectory phase occurs before any signs or symptoms appear.
2. Trajectory onset phase- occurs when signs and symptoms first appear and includes the diagnostic period.
3. The crisis phase -occurs when a potentially fatal situation arises.
4. The acute phase- this comes after the crisis phase and refers to the time when the patient’s symptoms can be managed with a prescribed regimen.
5. Stable phase – this phase begins once symptoms are under control.
6. Unstable phase – when the patient’s symptoms are not controlled by the previous regimen.
7. Downward phase – characterized by progressive mental and physical deterioration
8. Dying phase – refers to the weeks, days, or hours before death.
In the trajectory model, the nursing process
The chronic illness trajectory framework developed by Corbin and Strauss consists of six steps:
1. Determining the trajectory phase.
2. Identifying problems and setting goals
3. Making plans to achieve objectives.
4. Identifying factors that help or hinder goal attainment.
5. Intervention implementation
6. Assessing the efficacy of interventions
