Ten Step Ethical Decision Making Model
This model is based on and combines the work of others, including F.G. Reamer, Ron Hamel, and Mary Carlsen
1) Gather relevant facts and information

Clinical Mental Health Scenario
You are a MSW providing psychotherapy in the context of a DBT-based program for young adults with a history of trauma, self-injurious behavior, and other relationship and life-interfering behaviors. Part of the therapy involves clients completing “diary cards” each week which detail instances of the targeted behaviors, and the skills clients attempt to use to cope with urges to engage in these behaviors.
One of your clients is a 19 year-old non-binary person named Finn. Finn has a history of anorexia and bulimia, as well as cutting. They have in the past been hospitalized for both of these issues. They stated that the hospitalization for cutting was “an accident”, and they consider themselves to be “an expert” on how to cut without endangering themselves. Finn lives with their single mother, who has written you a letter stating that, while she loves her child dearly, she has told them that she will kick them out of the house if they cut or engage in behaviors related to the eating disorder again, because she has reached the end of her rope. The mother was abused by her husband, who also abused Finn. You have concerns about Finn becoming homeless, as they have little social support, and report that their circle of friends
has historically been organized around damaging and injurious behavior.
Finn has expressed a commitment to the work, and repeatedly said they are invested in finding new ways to cope. They make positive contributions to the skills training group, and you feel they have been making good progress in therapy, using the skills and supports (including appropriate after-hours support as needed) and their diary cards indicate no incidents of self-harm in the last 8 weeks.
As part of your own research, you have found and read message boards related to “pro-ana” culture and mutual support sites for people who cut. In doing so, you have read posts that contain details that make you suspect they were written by Finn. These detail recent incidents of both purging and cutting, and discuss how they keep these from their mother and their therapist. After reading these, your suspicions lead you to look at Finn’s public social media, where you see posts that deepen your suspicion that Finn has been lying in therapy.
