Sustaining Evidence-Based Practice
Introduction
Evidence-based practice (EBP) is known to improve the quality of healthcare and patient outcomes while decreasing healthcare costs. Still, the understanding of care based on evidence is sometimes far removed from clinical practice. Developing research has emerged related to moving beyond the barriers and toward implementation strategies in clinical practice (Melnyk & Fineout-Overholt, 2015). Nursing research increases emphasis on health promotion, technological healthcare interventions, and addressing contemporary healthcare issues. Today, EBP is a core competency for advanced practice nursing (APN). The purpose of this document is to explore methods that translate evidence into practice and contribute to the sustainability of EBP in the clinical setting.
Discussion
Magnet hospitals must encourage quality and communicate best practices throughout nursing, which are achievable only through the pursuit of an evidence-based practice environment. The measurement of outcomes paired with rigorous research results is the best organizational policies to guide the highest quality of care (Melnyk, Overholt, & Ford, 2011). Melnyk (2015) outlines the strategies used to transform the clinical environment. This student will use this outline as a practice guide. A written summary will be created describing desired accomplishments. Then, creatively capture the attention of staff and stakeholders by using real clinical scenarios to stress the need for practice change. To generate long-term change, the APN needs to focus on two or three attainable goals at a time. Administrative support is a necessity for sustainable change, so a presentation outlining the need for changing the culture into EBP is essential. Creating short-term change is not the challenge, but sustaining the change long-term is difficult. To maintain an evidence-based environment, establish a leadership team that is passionate about EBP. This team must include clinical experts as well as evidence-based practice mentors. The APN must assess and eliminate barriers to EBP implementation. Common barriers include inadequate knowledge and skills, weak belief about the value of EBP, poor attitudes towards EBP, lack of EBP mentors, social and organizational influences, and economic restrictions such as limited resources to support change (Melnyk & Fineout-Overholt, 2015). One study noted the tendency to underestimate the complexity involved in collaboration among stakeholders. Potential barriers to successful collaboration include lack of trust and deference, inequitable allotment of power, conflicting priorities and beliefs, conflicts over funding, and time utilization (Green et al., 2015).
This student believes that education is the catalyst for moving evidence into clinical practice. The APN needs to engage staff and stakeholders at all levels. Educational sessions are rotated by days and times to reach as many people as possible. A database will track attendees of educational offerings, and a volunteer may educate clinicians who missed the original in-service. Posters and buttons designed by staff will offer visual triggers to remind them to implement new policies (Melnyk et al., 2011).
Studies indicate training programs such as EBP journal clubs is a strategy to sustain EBP in the clinical setting (Farokhzadian & Poorchangizi, 2017). The session objectives are that learners will be able to (a) appraise research based on the level of evidence, (b) summarize findings based on levels of evidence, and (c) translate evidence-based findings into practice. Participants are required to complete journal article readings and attend a one and half hour session and evaluate the activity. Participants receive two contact hours for involvement. A library liaison is available during educational offerings as well (Gardner, Jr. et al., 2016).
Facilities need to be encouraged to adopt a model to guide and reinforce the creation of culture to sustain EBP. One model designed for EBP is The Advancing Research and Clinical Practice Through Close Collaboration (ARCC) model. It serves as a practical guide for institutions striving to promote EBP in the clinical setting. There are four underlying assumptions of this model.
Conclusion
In conclusion, the APN is the primary gatekeeper of EBP for the profession. They need to focus on what happens in clinical practice and look for common barriers that interfere with EBP (Bianchi et al., 2018). Leaders who have had success in EBP have a responsibility to share their wisdom by mentoring others.
