How is this agency/organization funded?
The health organization is funded through Medicare and Medicaid support. The organization receives Federal and State of Michigan funding since all of our patients are court-ordered to receive treatment at the facility. The health organization has some patients that have private insurance, and it can receive reimbursement for the treatment of the patients.
How are funding priorities set?
The health organization received federal dollars for Covid-19 and spent the funds on addressing Covid-19 pandemic issues such as surveillance items. The organization spent the money on capital purchases for ligature-resistant furniture and bathroom designs. The allocation for the financial year 2021/2022 was a priority due to the risk of infection and the safety of patients. Patient safety takes precedence as well as any Joint Commission or CMS citations that need to be addressed.
If a PHN feels another public health issue needs to be considered a priority and it is not on the agency’s/organization’s agenda, how is it handled?
The issue of the PHN would be reviewed at one of our employee open forums. If the Hospital Director felt it warrants further exploration it would be proposed at our Executive Management Committee by the person who feels it needs to be a priority.
Has the mentor had the experience of needing to stand up for an issue? Explain.
The mentor has been passionate about offering different forms of protection for employees. The respiratory Program at our facility per OSHA guidelines indicates that employees who cannot fit N95 should receive a different form of protection. PAPR training should be included and all employees should be trained on how to wear a PAPR in place of wearing N95. The Respiratory Program Administrator does not offer the appropriate innovation such as alternatives for N95. I am currently researching best practice guidelines and CMS/Joint Commission requirements to support the initiative.
How does this agencies/organization get community input on priorities?
The facility is a state hospital, one of the five and we have a bureau director of all the hospitals who consults with a director of public health coordinator. Input is also received by the Director of Infectious Disease as well as other government positions that are a part of community outreach and safety.
How has or does this agency/organization influence funding decisions?
The facility has the highest influence if it keeps us in compliance with Joint Commission or CMS.
Implications of funding on public health nurse leaders and organizations working to address public health issues.
The funding has an impact on the public health nurse leaders since it affects their capacity to address emerging issues in the health facilities. For instance, lack of funding can demoralize the public health nurse leaders since they have no resources. Demotivation can affect innovation and capacity to address emerging issues. During the pandemic, the lack of sufficient resources to hire additional staff and equip hospitals adequately was a great source of demotivation.
Allocation of sufficient resources annually is critical for organizations working to address public health issues. It helps organizations to invest in evidence-based practices in the healthcare environment. Research-based and patient-based care affects the ability to deal with emerging issues such as hospital-acquired infections, shortage of staff, and reducing medical errors. The implications are vital in boosting the quality of patient satisfaction and patient outcomes.
