Week 10: Components of Evidence-Based Practice: Grading the Evidence

What grade would you require of evidence to justify a practice change? Must it be an A or a solid 100? Would B+ or, say, 90, be enough? How would you attach a grading equivalent to the evidence you have appraised?
This final component of evidence-based practice does not require literally assigning a grade. However, it does involve evaluating what level of evidence in quantity and quality can justify a particular practice change, and the time, effort, and expense of a quality improvement initiative. Each situation and grading scale may be different depending on the specific practice problem, the potential outcomes, the urgency of the problem, and the attitudes—encouraging or uncompromising—of the stakeholders. Introduction:

Evidence-Based Practice (EBP) is a fundamental approach in nursing that incorporates the integration of the best available evidence, clinical expertise, and patient preferences to inform healthcare decisions and improve patient outcomes. In this discussion, I will present the results of my literature review and the connections to my practice problem. Additionally, I will elucidate my synthesis of evidence to support a practice change initiative, emphasizing the need for this change in a specific nursing context.

Results of Literature Review and Connection to Practice Problem:

My literature review focused on identifying relevant scholarly articles that addressed my practice problem, which is the prevention of pressure ulcers in elderly patients with limited mobility in long-term care facilities. The review process involved the Assessment of articles for their quality, relevance, and applicability to the practice problem.

Through this extensive review, I uncovered several key findings that are directly related to my practice problem. One crucial study (Smith et al., 2021) explored the effectiveness of a specialized pressure-relieving mattress in reducing the incidence of pressure ulcers among elderly patients. The results of this study demonstrated a significant decrease in pressure ulcer development among patients using the specialized mattress compared to standard mattresses. Synthesis of Evidence for a Practice Change Initiative:

Based on my literature review, I have synthesized the evidence to support a practice change initiative. Comprehensive Skin Assessment Protocols: Developing and implementing standardized protocols for frequent skin assessments will aid in early detection and intervention for potential pressure ulcer development.

Nursing Staff Education: Conducting regular training and education sessions for nursing staff on pressure ulcer prevention techniques and best practices will empower them to deliver optimal care to patients.

The Need for a Practice Change Initiative:

Pressure ulcers in elderly patients with limited mobility pose serious risks to their health and quality of life. Research has shown that these wounds are associated with increased morbidity, prolonged hospital stays, and higher healthcare costs (Johnson & Williams, 2016). Given the substantial impact of pressure ulcers on patient well-being and the burden they place on healthcare resources, a practice change initiative is essential to address this issue effectively.

The proposed practice change initiative is not only supported by robust evidence from reputable sources but also aligns with the principles of patient-centered care and the goal of optimizing patient outcomes. By implementing evidence-based interventions to prevent pressure ulcers, nurses can play a pivotal role in enhancing the quality of care delivered in long-term care facilities.
