Clarifying Connections: Practice Problem, Evidence, Changing Practice

Post an explanation of the results of your literature review and the connection to your practice problem. Then, explain your synthesis of evidence on which to base a practice change, and the need for a practice change initiative. Be specific and provide examples.

Week 10: Components of Evidence-Based Practice: Grading the Evidence

What grade would you require of evidence to justify a practice change? Must it be an A or a solid 100? Would B+ or, say, 90, be enough? How would you attach a grading equivalent to the evidence you have appraised?
This final component of evidence-based practice does not require literally assigning a grade. However, it does involve evaluating what level of evidence in quantity and quality can justify a particular practice change, and the time, effort, and expense of a quality improvement initiative. Each situation and grading scale may be different depending on the specific practice problem, the potential outcomes, the urgency of the problem, and the attitudes—encouraging or uncompromising—of the stakeholders. Each situation and grading scale will differ depending on the specific practice problem, the potential outcomes, the urgency of the problem, and the stakeholders’ attitudes (encouraging or uncompromising).
This module is completed this week with a critical analysis of the evidence from your literature review. The strength and substance of what links your practice problem, evidence to address it, and the need for a practice change initiative will be yours to explain and grade in both your Discussion with colleagues and your module Assignment.
This week you complete this module with a critical assessment of the evidence from your literature review. Discussion: Clarifying Connections: Practice Problem, Evidence, Changing Practice
Your literature review should be completed at this point in the module. Working from that assumption, you will use this Discussion to share your preliminary outcomes and analysis from your literature review with colleagues for their comment. You may choose to incorporate feedback from peers before submitting your Module Assignment on Day 7, although that is not required. The aim of this Discussion is to support you in providing the kind of analysis, Assessment, and synthesis of evidence to a group that would be involved in presenting a quality improvement initiative to stakeholders.

To prepare:

• Review the Module 4 Learning Resources with guidance for analyzing and synthesizing evidence from your literature review.
• Complete your analysis of outcomes and synthesis of evidence to inform a practice change.
• Consider the linkage between your practice problem, evidence to address it, and the need for a practice change initiative.
• Assess the strength of this linkage and how to present it to colleagues.
With these thoughts in mind …

ASSIGNMENT

Post an explanation of the results of your literature review and the connection to your practice problem. Then, explain your synthesis of evidence on which to base a practice change, and the need for a practice change initiative. Be specific and provide examples.

Clarifying Connections: Practice Problem, Evidence, Changing Practice in Nursing

Introduction:

Evidence-Based Practice (EBP) is a fundamental approach in nursing that incorporates the integration of the best available evidence, clinical expertise, and patient preferences to inform healthcare decisions and improve patient outcomes. In this discussion, I will present the results of my literature review and the connections to my practice problem. Additionally, I will elucidate my synthesis of evidence to support a practice change initiative, emphasizing the need for this change in a specific nursing context.

Results of Literature Review and Connection to Practice Problem:

My literature review focused on identifying relevant scholarly articles that addressed my practice problem, which is the prevention of pressure ulcers in elderly patients with limited mobility in long-term care facilities. The review process involved the Assessment of articles for their quality, relevance, and applicability to the practice problem.

Through this extensive review, I uncovered several key findings that are directly related to my practice problem. One crucial study (Smith et al., 2021) explored the effectiveness of a specialized pressure-relieving mattress in reducing the incidence of pressure ulcers among elderly patients. The results of this study demonstrated a significant decrease in pressure ulcer development among patients using the specialized mattress compared to standard mattresses. This finding directly addresses my practice problem and provides strong evidence for implementing changes in mattress selection and usage in long-term care settings.

Synthesis of Evidence for a Practice Change Initiative:

Based on my literature review, I have synthesized the evidence to support a practice change initiative. The evidence suggests that the implementation of specialized pressure-relieving mattresses, along with regular repositioning of immobile elderly patients, can substantially reduce the incidence of pressure ulcers. Additionally, the adoption of comprehensive skin assessment protocols and the involvement of nursing staff in pressure ulcer prevention education have been associated with better patient outcomes (Brown et al., 2019).

It is evident that a multi-faceted approach is required to effectively address the problem of pressure ulcers in elderly patients. Thus, my synthesis of evidence proposes a comprehensive practice change initiative that includes the following components:

Adoption of Specialized Pressure-Relieving Mattresses: Incorporating specialized mattresses in the standard care protocol for immobile elderly patients can significantly reduce the risk of pressure ulcers.

Regular Repositioning: Implementing a schedule for repositioning patients, coupled with proper skin care, can alleviate pressure on vulnerable areas and further prevent ulcer formation.

Comprehensive Skin Assessment Protocols: Developing and implementing standardized protocols for frequent skin assessments will aid in early detection and intervention for potential pressure ulcer development.

Nursing Staff Education: Conducting regular training and education sessions for nursing staff on pressure ulcer prevention techniques and best practices will empower them to deliver optimal care to patients.

The Need for a Practice Change Initiative:

Pressure ulcers in elderly patients with limited mobility pose serious risks to their health and quality of life. Research has shown that these wounds are associated with increased morbidity, prolonged hospital stays, and higher healthcare costs (Johnson & Williams, 2016). Given the substantial impact of pressure ulcers on patient well-being and the burden they place on healthcare resources, a practice change initiative is essential to address this issue effectively.

The proposed practice change initiative is not only supported by robust evidence from reputable sources but also aligns with the principles of patient-centered care and the goal of optimizing patient outcomes. By implementing evidence-based interventions to prevent pressure ulcers, nurses can play a pivotal role in enhancing the quality of care delivered in long-term care facilities.

Conclusion:

In conclusion, my literature review has provided significant evidence to address the practice problem of pressure ulcers in elderly patients with limited mobility in long-term care facilities. The synthesis of this evidence has led to the development of a comprehensive practice change initiative aimed at preventing pressure ulcers and improving patient outcomes. By embracing evidence-based practices and committing to continuous quality improvement, nurses can make a profound impact on patient care and safety.
